Your needs may connect you with VA health care, Home and Community Based Services, Community Care, caregiver programs, or other Veteran resources.
VA Health Care Eligibility
Veterans may qualify for VA health care based on their military service, discharge status, service history, and other eligibility factors. Veterans who served in the active military, naval, or air service and did not receive a dishonorable discharge may be eligible. Minimum active-duty service requirements may apply depending on when you entered service.
Expanded eligibility also applies to many Veterans who were exposed to toxins or other hazards during military service, including Veterans who served in Vietnam, the Gulf War, Iraq, Afghanistan, or another combat zone after September 11, 2001.
Applying for VA Health Care
If you are not currently enrolled in VA health care, completing your VA health care application is an important first step. Veterans can apply for VA health care online, by phone, by mail, in person at a VA medical facility, or with assistance from an accredited representative.
Before applying, VA recommends gathering information such as your military discharge documents, including your DD214 or other separation records, Social Security information, insurance information, military service history, and information about possible exposure to toxins or hazards.

Understanding VA Community Care
VA Community Care allows eligible Veterans to receive approved health care from community providers outside of VA facilities. To qualify for Community Care, a Veteran generally must be enrolled in or eligible for VA health care and receive approval from their VA health care team before receiving care from a community provider.
Community Care eligibility may be based on factors such as the availability of a needed service through VA, wait times, travel distance, quality standards, or whether the Veteran and VA provider determine that receiving community care is in the Veteran's best medical interest.
Important: Eligibility for VA health care does not automatically mean every Veteran qualifies for every Community Care service.
Getting a Community Care Referral
Before scheduling covered care with a non-VA provider through the VA Community Care Network, Veterans generally need a referral and approval from their VA health care team.
The process typically includes:
1. Find an in-network community provider.
You may search for a provider yourself or ask your VA health care team for assistance.
2. Ask your VA health care team for a referral.
VA will review your request and determine whether you are eligible for Community Care and the requested service.
3. Schedule your care after the referral is prepared.
Depending on the referral, you or your VA team may schedule the appointment.
4. Review your VA authorization.
Your authorization identifies the care VA has approved. Always confirm that the service you are receiving is included in your authorization before care begins.
VA states that the referral preparation portion of this process may take up to 14 days.
Home & Community-Based Care Options
VA offers several Home and Community Based Services designed to help eligible Veterans receive care while remaining in their homes and communities whenever appropriate.

Care Admirable Home Care Provides**
The appropriate service depends on the Veteran's individual needs, clinical eligibility, program requirements, and availability in the Veteran's area.
Homemaker & Home Health Aide Care
The VA Homemaker and Home Health Aide program helps eligible Veterans who need assistance with personal care and activities of daily living.
A trained home health aide may come into the Veteran's home and assist with everyday needs, helping Veterans remain safely and comfortably in their own homes.
This program may be appropriate for Veterans who need personal care assistance, experience isolation, or have a family caregiver experiencing caregiver burden.
Services are provided through organizations that contract with VA and may be used alongside other Home and Community Based Services. Enrolled Veterans may qualify when they meet Community Care requirements, clinical criteria, and the service is available in their area.
Need help with everyday activities at home? Ask your VA social worker or care team whether Homemaker and Home Health Aide services may be appropriate for you.
Asking About Admirable Home Care
Veterans receiving authorized Community Care may have an opportunity to receive services from a provider within the VA Community Care Network.
If you are interested in receiving your approved home care services through Admirable Home Care, speak with your VA health care team or case manager.
You may say:
“I need assistance with care at home and would like to explore my eligibility for VA-authorized home care services. I am interested in Admirable Home Care, LLC as my community provider. Can you determine which home care program I may qualify for and whether Admirable Home Care is available for my authorized services?”
VA allows Veterans to search for Community Care providers and request a referral to a community provider. The Veteran's VA health care team must determine eligibility and approve the care before covered Community Care services are provided.

Respite Care for Veterans & Families
Caring for a Veteran is meaningful work, but caregivers also need time to rest, take care of personal responsibilities, attend appointments, run errands, or travel. VA Respite Care provides short-term care for eligible Veterans so their family caregiver can take a temporary break.
Respite may be provided in the Veteran's home, through an adult day health program, or in certain nursing home settings depending on the Veteran's needs and available services.
Respite Care may also be combined with other VA Home and Community Based Services. Eligibility is based on clinical criteria and service availability, and Community Care eligibility is also required when respite is provided through a community organization.
Support for Family Caregivers
Family caregivers are often an essential part of a Veteran's ability to continue living safely and independently. The VA Caregiver Support Program provides education, resources, support, and services for caregivers of eligible Veterans enrolled in VA health care. VA caregiver resources may include skills training, individual coaching, group support, peer mentoring, self-care resources, telephone support, referrals, and additional services through specific caregiver programs.
VA operates both the Program of General Caregiver Support Services and the Program of Comprehensive Assistance for Family Caregivers, each with different eligibility requirements and benefits.

Finding the Right Care for Your Needs
You do not need to know the name of a VA program before asking for help. Start with the assistance you need.
I need help with personal care. If you need assistance with bathing, dressing, eating, toileting, moving safely around your home, or other daily activities, ask your VA care team about available personal care and home support programs.
I need nursing or therapy at home. If you need wound care, nursing services, physical therapy, occupational therapy, speech therapy, or other skilled services, ask about Skilled Home Health Care or other clinical home-based programs.
My family caregiver needs help. If your caregiver needs temporary relief or additional support, ask about Respite Care and the VA Caregiver Support Program.
Staying in my home is important to me. Tell your VA care team that remaining at home is one of your care goals. VA's Care at Home and in the Community Decision Aid can help Veterans and families consider needs, preferences, available support, and possible long-term care options.
